Application for Library Membership and extending the membership
Sabaragamuwa University Academic/Non-academic staff members can use the following form to obtain the membership of the University library. For students, it is not necessary to submit a separate application to obtain their membership and the University registration entitles to obtain the library membership as well.
Form for Requisitions
Following forms can be used to recommend a new title (book, journal, articles etc.) to the library. The request should reach the library through the head of the corresponding department and the Dean of the respective faculty.

Lost Books & Compensation
One can use the following form to recompense for the lost books.
No Claims Report
Following form can be used to ensure whether a library user has returned all the library books and has no claims to the library.
Inter Library Loan (ILL) Request Form
One can use the following form to request a book (which is not available in the library network of the Sabaragamuwa University) from another University library of Sri Lanka.
